4. You can also place your order using the order form as follows:

• Download order form and fill Product No’s and descriptions according to our catalog or website, and we will fill the rest of the part. Please don’t forget the shipping and billing address.

• Please add any specific requests and/or comments in the note area.

• E-mail your order forms back to: diapads@gmail.com;or fax it to 888-811-4563. 

• We will contact you for payment options. Please DO NOT give your credit card

information on the order form and in your e-mail.

4. Your order will be delivered to you via USPS priority or FedEx ground, or  other means that you specify.
